I do all of the above... I usually put the coins in flips, then I bubble wrap them, tape them to the inside of the envelope/package, and I also tape the envelopes/packages after they've been sealed. I'm very thorough, and often get the comment "very well packaged/protected" when I get feedback.
The slit envelope seems to be an isolated incident which likely happened close to the destination. My guess is that it's probably someone in charge of picking up the mail at certain drop off boxes who is responsible for the lost coins.
