Sloppy Serial Digit In Both Serial Numbers

I've seen many notes in which a particular serial number digit place is blurred or overinked or otherwise sloppy. It happens. Sometimes, however, the same digit in both serial numbers are similarly sloppy, such as the first 9 of this note. I don't understand how this can happen with the frequency it does. The two serial numbers are printed concurrently by two separate counters, right? Why should a particular digit in both counters be messed up at the same time at different places on a note?

AFAIK a given counter mechanism does not make two impressions on the same note, so the smudged/overinked 9 was printed by two different counters. When two different counters exhibit the same problem it makes me suspect a defect in the counting mechanism or the ink supply.
