It is a 1966 quarter that is dark all over both sides, and this is an obvious PMD coin. It looks as if it went through a fire. The strangest feature is the large bulge on the reverse.

Yes exactly--this is a so-called campfire coin. The high heat causes that weird bubbling between the clad layers.
