1946-S Jefferson Nickel With Strike Through, Bag Mark, Lost Lamination, Planchet Flaw?

I bought this coin months ago, but I am just now clearing smaller purchases off my desk. What is this odd cavity on the top of Monticello? There is a similar, smaller area on Jefferson's cheek. I don't see metal pushed up around the edge, although many bag marks don't displace major metal. Perhaps it was a flawed planchet or a planchet with a lamination defect? Also seems possible that it was struck through some debris in the chamber. Thoughts?

How's the weight @brand. If a chunk that big fell off the planchet, then I'd expect you could measure it. Otherwise, strike-through seems most likely to me.

It is definitely incuse. The bottom of the hole is slightly uneven. The mass is 4.94g, which is 99% of nominal. However, I don't know how much a grain that tiny would really weigh.

One big strike against the lamination possibility is that you can usually see other striations and uneven toning in the surface due to poor metal mixing. I don't see any of that on this nickel.
