Is this a lamination error and possibly something else?

Hello everyone. I joined to post this http://i.imgur.com/YJKie.jpg. It is a quarter that I recently obtained that caught my eye. I am by no means knowledgeable about errors, but particularly the pattern in the lower part of Washington's hair resembles some other lamination errors I saw online.

The reason I am not sure about what else might be going on here is because there is a noticeable dent in the front of his neck and another one that is less apparent in the picture, toward the back of his neck.

The two holes are the most striking deformation, and they can be seen on the eagle side as well. On the eagle side, the two hole areas look similar in pattern as the lower part of Washington's hair. This is noticeable on the left wing of the eagle and the body.

It is entirely possible that I could be completely wrong about this whole thing, so that is why I have come to people that really know their stuff. Any insight into this is greatly appreciated. Thank you all in advance.

First:you did a very good job with your description.
Second:all I see is damage but the image is a bit small,I could be just missing the lamination.

Well to the CCF forum:

The coin has two indents on the obverse and two bulges on the reverse. Someone altered this coin. Punched it with something. Sometimes even nice coins get ruined by anothers hands.

Thank you. I just attempted to scan it at a better quality but it seems that is the best it will do to allow the defects to be seen.

Thanks coop. That did occur to me. The dents are oddly shaped, which makes it more likely that it was damaged. Since I was unable to explain the "bubbling" pattern, I thought I should post it here.
