Whilst I am not authorised to officially speak on behalf of the royal trout mint (RTM) it would appear to be a genuine error on their behalf.

Maybe the RTM CEO could chime in for confirmation of whether or not their um... machinery... caused this.

Yes that would be one of the hole saw errors.
Or it may even be a proof lathe error, VERY RARE
