Where were these coins minted? What was their quality control guidelines for errors?

It is not a coin, just a silver round that could have been made by a number of bullion manufacturers. I doubt that there were any "quality control guidelines" other than ensuring that it contained 1 troy ounce of silver since everything else is irrelevant.
