it is a form you use to fill in the details when you send coins to ICCS. I have used my own numerous times... never had a problem.

Thanks coingirl. So this form is basically used for what? Grading of coins, report error coins, sorry I'm new to this so have no clue. Thanks

barriecarson, until you have enough posts (I think 50?), we cannot contact each other on this forum. You would have to ask the forum administrators directly for permission, to get my email (or vice-versa). Or, I could convert the PDF of the ICCS submission form to a low-resolution JPG image, and post them here? I know that ICCS and Brian/Scott Cornwell would not care - it would potentially drum up additional business for them, but the forum here may be a little uncomfortable with a post like that (i.e., copyright issues and the like). If you ask nice enough, the former option is likely possible.

I've looked all over and couldn't find a good enough image of the ICCS forms to print out for myself. Can someone point me in the right direction or PM/email me them? Thanks!

Thanks, I have seen that one. It is somewhat out of date re: pricing and who knows what else. There is also a second page with just submission lines that I also need, which isn't on that site.

Hey guys on the ICCS form. The "owner's value" field.
Can that be left empty? Or you suggest an value that you estimate?
Is it required.

The owner's value field can be left blank, but only if you are declining insurance on the return shipping. That is the value they will insure the coin for.
