Damaged coin. (Never are mint errors) These were altered post strike after the strike.

Yes, post-strike damage. Perhaps a hand punch, perhaps from a bb fired at close range.

Pretty good description of what you're seeing. I can't see the area under the chin or above 49 clearly enough to confirm or make suggestions so I'll say congrats.

Maybe pellet gun will do that, mine is almost as powerful as my 22 cal.

Well, not seeing a lamination 'above the 48'. Just behind the ponytail is part of the design elements.
