Not to split hairs, but a can opener typically has a blade on one side and a toothed gear on the other. Unless you are talking about one of those fancy ones that hug the rim and cut the sides (a.k.a. the manual can openers that I can never get to work right).

OK good news..... mystery error solved...mint sealing machine damage (MSMD)

and

I got the quarter out of my can opener... People don't try this at home...

Tbh, I am still trying to understand how a cellophane sealing machine generates enough force to rip the surface of a coin with grooves.
