It does make me wonder how many coins where struck in between each one of those different stages.

There have been a few BIE's presented like the first image here. Could we call the first one a "Pre-BIE" and the piece of the die had not dropped out yet?

This is a good indicator of them forming.

Super! Finding multiple versions in a single roll shows how quickly these develop. A roll is probably taken from a large bag, so these probably happened within 2k strikes or so.

I liked it so much I made an animation from the 3 photos. Couldn't do all I normally do given the very tight size requirements though...

I did check Cohen and I don't see this one listed. The relevant markers are the partially-filled lower B, and the die crack on the forehead. There are a couple BIEs that are similar, but none have these markers and Cohen would not have missed them. So I think this is a new discovery piece!

I made a larger and slower animation that Bobby is graciously hosting. I decided to only use the first and last image since the lighting is more similar and the middle stage is almost as well developed as the last.

That is awesome (the large animation and the discovery news)!

I do like how the smaller animation with the 3 photos shows more movement, an almost tidal-like fluidity.
