2012-D Jefferson Error Help

Hi Guys. I am not an error coin expert. I got this coin in change today. Lots of rim damage top and sides. Lots of gouges and holes mostly on the obverse. A nice nickel size scratch across the forehead and an obvious (raised metal) die break on the obverse between the one and zero in the date. What do I call this thing and what would have caused it? Thank you for your help.

I'd call it a PMD. Post-Mint Damage.
The "die crack" seems to be a scratch. PMD.
The big hit on the edge and a partly cut off edge. PMD.
I feel bad for this coin - so much PMD!

If raised, the width tells me it's more likely a gouge(learned from coop)
But the amount of PMD would negate any collector value.
