The second strike is of a 12-sided penny from 1982-1989. This points to it being a PMD HAD IT BEEN REVERSED. Thus, I conclude a mint-employee brought it in and did that either on accident or on purpose.
I agree with this being PMD, altered using soft dies. Dollarman already pointed out that the raised "rim" on the second strike can't happen without a collar. A struck through cloth will also leave it with a cloth pattern and more blurred devices on the coin, and not a "second strike" as clear and sharp as what is seen here.
