PMD, looks like someone took a socket, put it on the coin and hit it with a hammer. Probably why you don't see the damage on the reverse could be it was placed on a piece of leather before it was hit with a hammer. That's just a guess.

Circle is too concentric. Looks like it was turned down on a lathe. I'm sure others will chime in.

Either way it is PSD. Nothing in the striking process could alter a coin like that. (I'm leaning more towards the machine shop lathe theory. the socket theory would have made a pushed out area on the reverse of the coin)
