Just damage. We call these Parking Lot or Lawnmower Coins as they look like they have been repeatedly run over or caught under a lawnmower. I've also seen damage like this from a coin caught in a garbage disposal.

It is a 2006 North Dakota quarter, reverse.
Rotated 90 degrees clockwise.
