The surface of the 4 looks shaved and flattened but how does that explain the missing left and bottom areas? The fields around those areas seem to show die wear lines which you wouldn't expect with damage or grease fills.

Looks like damage to the 4 to me. There is die flow lines present. Might just be coincidental.

Hard to say sometimes what happens to a coin after it leaves the mint.
If it is not a Grease Filled Die then it is damage
of some sort.
