2004 D Nickel, A Little Fun Going On With This One

Going back thru some 2004's and found this. A little extra going on over the "R" in America, but the "A" in States caught my eye first. The area to the left is raised. Tried moving the light source a little to make it stand out. None of the other letters have this issue. Always looking for a little education!

Maybe Machine Doubling on the A? There's a Die Chip on the R.

The "R" looks like it has a die chip.
Is that area left of the "A" raised or incuse?

Took another look with the 20x loupe to get a good look at it. Here is a pick at a better angle and a lot of magnification. I was looking straight down the edge of the "A". Apparently I have a little die break and PMD or a little strike through?
