How can I tell if someone scrubbed this token clean "with Brillo", as someone else had suggested in a former post, OR if the blank planchet just started that way, as another someone had suggested in the same post?

Considering all the pimples in the fields and lack of detail on the devices, In my opinion it appears to be cast.

I do not know the type. But I see nothing there that could not be the product of minting with crude dies. The parallel lines can be from the die. Does not look like scratches on the coin. The excess metal spots around COLUMBIA and in the bust can be from a rusty die.
