At first I would say it was a flaw in the planchet but as I thought about it I am leaning towards a cracked die - it just looks like that is the most plausible explanation. But I am far from knowledgeable on the topic.

I picked this up at my LCS but can't find it in my catalog or figure out a date. Any help is appreciated!
It measures 34 mm.

Approximately 32 mm across and very thin. Appears to be silver.

Well - you can start with Y# 135. There are a number of further varieties 135.1 - .10 in my Krause. It would have to have a reeded edge to be just 135 and yours looks like it has a plain edge.
My best guess is 135.1 with a plain edge
.2 Small Manchu words in center of obverse - hard to tell what they mean - the pictures are poor
.3 talks about large Manchu characters in the obverse center - hard to tell but your don't seem large.
.4 No description is given for .4 - Krause is not perfect.
.5 Cloud above T looks like letter 3 - I don't see it.
.6 C of CASH is G - nope.
.7 Thin tailed Dragon - nope
.8 Fewer clouds around dragon, pearl smaller - don't think so
.9 Small rosettes on either side of dragon - but all the pictures have rosettes so who know what they mean
.10 large oblong rosettes - nope

As has been suggested before it could be a front for an illegal transaction - so a way to spend money electronically for something else - drugs, stolen goods, etc. Just repeating a prior suggestion I found interesting.

Looks like a horizontal fold and a little rough around the edges. If there is a vertical fold as well I would say VF - if not then EF.

Machine doubling in general (normally 1/16 of an inch or so) would not be worth anything but 1/4 inch is a lot - I would also like to see a picture

As to the 1953 set being BU (as opposed to PL) - I think you are correct but I don't really know for sure. I have not seen one in a Mint holder.

In my book this coin type has 6 designations - LC-60A, 60B, 60C, 60D, 60E, 60F and then varieties with each designation.
Yours looks like LC-60E-1 because of the big double chin and the ten strings.
I would probably grade it EF.
