If there is only only, the coin was altered. If it were from a damaged die there would be several of them. But we see stuff added to coins all the time.

Looks like two light directions. The light is from the right, on the blue arrows.

I suspect someone used a vibrating engraver to crudely carve an ampersand into the coin's surface.
Error coin writer and researcher.
