Forget "notching." Or, rather, that's just one small piece of the diagnostics. It's a fooler every time when it causes you to take your eyes off the ball. Here, see this diagram, and think of what you're looking for. These are two images on the die, itself, slightly ajar, but right smack up against each other. They're both FULL images. It's only one image, the one formed by the second punch, is covering up much of the first punch image, that part of the first punch that's punched over by the second punch, and, as as a result, we can't see.
