To me it looks like it was place between two/three cents and two pieces of wood then then smacked with a large hammer. Transferring the devices to the victim coin. Just damaged on a remodel/construction site. All coins were damaged by this process.

The clue? The reverse/mirrored letters. They are that way because they came from another coin. If they were legit, they would not be mirrored:
