Just damage making it appear that way. This coin has damage all over the front and back, it may have spent a day or too in a parking lot getting run over.

I see the obvious damage but the "S" wouldn't be thin on the inside, thick on the outside from damage?

It is just damage which caused metal displacement, the same type of contact marks can also be found on both sides of the coin.
