This is a very interesting and I think unique occurrence...
You can see the jagged line where the device "E" joins the coin...
Anyone seen this before on an uncirculated coin?

It might be a lamination or coin damage. Can't tell for sure.

Looking again at the second image, it does show more of a cut into the coin issue. So it is PSD.

I can't tell for sure, but would lean more toward PMD because of what looks like a hit to the rim in the same area.
