I have this 2014 Utah quarter and I don't know if this is and error and what the error name is called

This coin is out of round but has the reeding which means that it didn't leave the mint looking like this. My vote is for this to all be damage.

The flattening on your coin coupled with the vertical groove on reverse is an indication of an immense pressure that was mannualy applied to this coin by a Vise, it's now considered a damaged coin. PMD
