PCGS - That blob you see on the obverse of this Washington quarter? That's a Cud, an error made by a broken section of the die. This coin is also an uncentered broadstrike, as it was struck imperfectly outside of its collar. In PCGS MS62, this piece serves us double the errors and double the fun this Mint Error Monday. What are the most errors you've seen on a single coin?

You forgot to mention the several struck-in-reeding impressions on the reverse. It's also an off-center strike, since peripheral letters are cut off on both faces.
Error coin writer and researcher.
