I wasn't able to arrive at a definitive diagnosis for this specimen. It's clearly not a tilted die error or a tapered planchet error. The only thing I could think of is that the planchet was characterized by uneven hardness. Softer areas expanded more easily and contacted the collar more forcefully, generating a taller edge. Harder areas resisted expansion, resulting in weaker contact with the collar and a vertically shorter edge. This is, of course, completely speculative.
