Limit of 1 per household.

Like the folks are saying on your FB page, the form is not working. The page is blank.

Technical difficulties that I assume will be corrected soon.

I was wondering about this, because the catalogue I just received from the RCM said to get these collector cards to go to an address on the actual website, but the web address given made no mention of them.

That requires changing security set up in FB and not something that is not recommended.

Incidently, the form now appears while using https and can be completed but it fails to submit. Maybe we'll get there soon.

Yes, we fixed the form so that it would work for both http or https settings :)
It was our first time setting something like this up in Facebook so lesson learned and all good now!

Strange, I just placed the order but noticed afterward that there was no place for my name, just email, address, city, province and postal code. Hope this won't be an issue

It all works now - both secured and unsecured.

However, chris86 is correct that there is no 'name' input.

Hopefully, the Mint does not include our e-mail addresses on the snail mail - that might be a little privacy issue.

I couldn't see the form yet since I'm at work and facebook is blocked but isn't the form a Facebook application? if yes, it's able to get your name from your account information... No need to input it!

I don't have a facebook account but I was able to fill in the information and I received the confirmation email. I used my Canadian address. I was wondering as well about the lack of name.

i just filled it out, everything OK, got my confirmation email. I had problem finding RCM on facebook. easiest way just click on the link that the RCM rep put on this thread.
