The 4 Looks like it is showing the same Machine Doubling from what I can see. Can you see striations in the area in question? If so it is Machine Doubling for sure.

If the 4 is MD and the MM is added separately, how does the MM get doubled by the machine too? Just curious.

Both are added to the die. The die is normal, but during the strike the dies moved and damaged the coin's devices.

The mint marks are not added to the coin, they are added to the die.

The coin is struck with the date and mint mark on the die. When struck they exhibit what's on the die, and the way the planchet was struck.
