Appears to be PMD. It looks like it was pushed down by the photo of the rim.

you will probably never know "with what".
something heavy could have fallen on it. soeone could have struck it with something either intentionally or accidentally....

"with what" probably dosent really matter much..

I am thinking being struck by a flat head screwdriver not it. Does not have any smooth indication where strike would have been. Looks like it has ridges inside.

Is the area near the rim bent out? Hard to tell what is opposite the area in question. The reverse opposite looks strong on that area, where if it were a pre-strike lamination it would have been weak. If it peeled off later, then the coin would be less than normal weight. If it is normal weight, then it is PSD. Hard to tell from images provided.
