1861-S quarter. Mintage 96,000. PCGS survival 60, none known in mint state. These coins saw hard usage and few survived.

Mine would be this 1858 proof quarter. Mintage estimates range from 80+ to 300; PCGS mintage estimate is 100, with a survival estimate of 80:

Wow! Although it's not as low as that, the lowest mintage I know off the top of my head for a coin I own is my 1883 proof seated half, with 1,039 struck.

Lowest mintage? Probably my 1867-S Half Dime with 120000.

Lowest survival rate? Low 200s.

Having some difficulty taking a good photo but I just acquired a 1880 Seated quarter that has a mintage of 13,600.
