I ran into this yesterday and found it interesting. This 1993 Dime as a radical misaligned obverse die strike. But they where not done messing up then they added the tilted partial collar to make it even more interesting. The reveres also has some "Very Deep Die scratches all over it"

A partial collar MAD coin with machine damage on the date/mint mark. There is also a miss match on the dies. The Obverse an EDS and the reverse LMDS. (Late Mid Die State)

That's a very efficient error coin, Russ. You can tick three different errors off the collection list with one coin. I'm guessing they were under time pressure from having had to stop to polish the reverse, and threw everything back together in too much of a hurry. Your coin might have been the first strike from the result, and I doubt it was supposed to leave the Mint. Or that it went unnoticed.
