Don't believe it is PMD. It appears to be a die chip as there is no scrap or damage around the chip.

From the image I can't tell if it is raised or incuse. I feel it is incuse because there is nothing that would cause to be incuse on the die in the mint process.

Interesting. It could be a die chip. What is the reverse looking like at the same spot?

You are sure it is secure on the head. (look around the base of the anomaly to see if there is a seam?) If there is no seam it must have been something that damaged the die. (which I feel is unlikely) The head is deep into the die and contact to the die like that would seem impossible. It looks too much like a device, rather than a die crack. Too much like a device than a die chip. so I'm not sure what happened to the die on that one?

Perhaps a touch of acetone to loosen anything that might be holding that piece in place if it's adhered, rather than something that happened at the mint.

It isn't a dropped letter,a dropped letter is always incused not raised.i was looking at the picture and it looked like there might be a die crack if so that could cause it

I can not see any lines or die cracks coming off this. It is stuck to the head as if it were pressed into or onto the head. I may have to send this one to the pros at Coneca or Wexler to see what this is.
