I would think that few individuals could sell to refineries directly, they would want to deal in much larger quantities than one would likely be able to provide.

I would think that one would have to sell to a middleman, who would pay quite a bit less than melt. He would then combine your coins with all the other silver he has bought into a pack large enough to be worthy of a refinery.
