I suspect it's not a die chip. I think it's a damaged die. Something got in the way during a strike and pitted the die. All subsequent strikes would have that characteristic until someone noticed it and changed the die. And I bet a whole bunch of cents got recycled.
Nice and unusual, but it does look like it is coming off of a die crack. But, I am familiar with all the technical processes and the "how" of a lot of these errors or varieties. It looks like it would be the 4mm requirements did you check with JC?

That's how I see it also CC: as bleed from a tiny die crack -- which I doubt I could have made that call were it not for lawest's excellent photography..
