I have a PCGS MS69 2001 Silver Eagle in a slab that says 2004. An obvious mistake from PCGS. Any extra value?

There is little if any added value for typing errors on the labels .

what I would be more concerned with is if the holder and coin has been tampered with ?

The holder can be straightened out by returning it to PCGS the coin can also be checked for accuracy of the grade .

Just sounds to me like a mistake in slabbing? In any case how did it become a 2001 when 2004 is on the coin

It's actually a 2001 dated coin in a 2004 Third Party Grading service holder or "slab".

I've seen State Quarters labeled as silver eagles before it's not really that big of a deal. I think PCGS will reslab it free of charge.

I have an error slab that worked to my favor. I bought a proof 1954 PCGS PR64RD penny off of ebay for $10. The sellers pictures were decent but I didn't pay attention to the date. Neither did he, so my 1954 turned out to be a 1951. Now I need the 1950 and I'm done with the proofs from 1950-1964.
