Is there any possibility this 1978 D penny was damaged during minting? If not, any idea what kind of damage could cause these unusual patterns?

Nope, that's Post Strike Damage, since it happened after it left the U.S. Mint. The metal has taken a hit, causing it to get pushed over and flattened.
