His ear is missing, there's just a divit there. What could have happened?

Looks like circulation/intentional damage affected the ear area.

The outside part isn't really that strong that year. (edited coin with an added mint mark. Just an example I had on my image host)

@cb2, as I don't see any evidence of a lamination or other planchet flaw, I agree that this is most likely just damage.
