Coin is in great shape other then that spot. It is caved in at that spot. Grease Filled Die or something else? Keeper? Thanks to all who reply

I see a lot of circulation scratches all around the bust area. My guess is that it stems from circulation wear. The ear is one of the highest points on the coin and will be one of the first places for circulation wear.
