Looks like post mint damage to me. The metal got stuck to the coin after it left the mint.

PMD from a hit, which pushed the relatively soft copper metal to form the (rough) shape of a D.

looks like a lamination issue with the raised area as a peel starting. I'm calling this an attached peel of a lamination.

@jk, perhaps you can post overall pics of the obv and rev of this coin? That might help us with deciding on damage vs. lam.
