I have a 1995 Washington quarter the has the words "quarter dollar on opposite side and other marks I can't identify.need help

It looks like a vise job to me - someone has pressed another coin onto the obverse of this quarter.

Just a damaged quarter, probably intentional.
It certainly didn't happen during the striking of the coin.

Yup, That's a vise job. Sorry 'bout that.
Can also be done with a sledge hammer, the product coin between two others.

Altering a coin doesn't make it a mint error. Note that both sides of the coin on that area, have been pressed in. On the quarter dollar area, there could have been a Grease Fill issue on those devices. They will be shorter because of a Grease Fill issue. (The deeper part of the devices of the die was filled, so only the bottom wider devices are seen.

If it were more grease filled it could look as bad as this coin:

Just a spender coin.
