Would this be considered lamination error? Any thoughts?

No, it looks like it is in a ring so it could be hung on a bracelet or necklace.

itzwert, it appears that your nickel is in a bezel or some other type of jewelry holder.

There is no loop or place for attachments. It's just rounded all around the rim and is peeling a little in one spot.

Probably used to be where it is pulled away a bit. Not an error, just PMD.

It's a bezel. That little lip on the bottom of the reverse is where the suspension loop broke off.

This piece should be overweight due to the added mass of the bezel. Depending on what it is made from, that part might be attracted to a magnet as well.
