I'm thinking it took a hit and pushed the metal down to that bump below the ear which I think you are looking at. It is definitely not an extra ear lobe.
Can you give us a full, clear coin shot of the obverse and reverse of the coin? What you are seeing as a doubled ear, along with other things may be from a die clash.
Thank you for the additional pics. I do not see anything else on the full coin shot to suggest a clash but still not 100% sure that it is not a clash. I do not feel it is a doubled ear though. It is a zinc cent with copper plating, and they are known for having issues between the zinc and plating that this could also be. I have pointed out the areas that I see that lead me to lean towards a rotated die clash. Here is a link for you to peruse about clashes.
