The 2x2 flip stated that this was "triple struck". What I can see is that the lower part of the Jefferson bust is incomplete, and there are faint raised shadow images above and on either side of the dome on the reverse.

Any insight as to what actually happened would be appreciated

It looks like a lot of die polishing happened on the obverse side
not really seeing any 'triple' struck on the reverse, but some radial flow lines from a tire die.

Is this your coin? If so, can we get some close-ups?

Is it me or does Jefferson look decapitated with their head floating.

pix are a little dark to see much detail, but I see *some* Machine Doubling and what appears to be an over-polished die, which makes the head appear to be 'floating', sort of like the 'floating' roof effect on some late '60s/early '70s LMC reverses.
