nickelsguy or SPP, could you explain how this one came to be as one would expect a full brockage to cover the whole surface of the coin but this one appears almost to be the size of a nickel or penny

Darryl that is a good question. In my error travels I have only seen one other 1999 mirror. A Dec coin if I remember correctly. It shows the same traits. My opinion will be shared once I part with the coin. Which will be soon.

What the heck Darryl. My research and testing shows that the brockage strike die was not a 25 cent planchet but a 10 cent planchet struck which then stuck to the hammer die........this is only supposition but it is the best I can come up with........I placed a 25 cent struck on 10 cent planchet on my coin......so close to fitting it is unreal.....I may try to get pics. If anyone knows of a 1999 July 25 cents struck on a 10 cent planchet I would love to see it.

Very Interesting ! I never gave much thought to the scenario above however it is plausible.
Almost looks like something Zonad owns.
There does seem to be expansion of the designs/devices in the original picture of the Brockage strike. Do you think that the "Quarter on a dime planchet" whacked a few other 1999 July quarters as well?
