Hello everyone. While sorting through some coins, I found a 1979-D Jefferson nickel that seems to have a RPM. Or is what I'm seeing caused by Die Deterioration? Thank you for any assistance offered.

The mint mark punch that was used might have gotten damaged from repeated use. The mint mark is also filled.

I found the same thing a while back. The notch in the lower left serif of the D is caused by a damaged mint mark punch. Links to Error Ref are included in my post:
