1510 K-G Hungarian Denar,King Wladislaus II. On the madonna side of the coin,it should have PATRONA. Instead,it has the PA from PATRONA and ISLAI from WLADISLAI which should be on the other side. On the shield side, instead of WLADISLAI it has WLADTRONA.

Back to the future! A 15100 K-G Hungarian Denar,King Wladislaus II. The other extreme;an extra zero in the date.

Those seem like major diecutting blunders- there's no way they didn't notice the error, but I guess they probably didn't want throw out a usable die.
