"SANCTVS PETRVS" obverse, which makes Papal States a good guess.

The shield looks like that used during the reign of CLement XII, and you can see the letters LEM at 7:00-9:00 in your second photo, so that gives a date range in 1730s.

Size is quattrino, though I havent found a match yet..

FWIW, both my wife (who was raised Roman Catholic) and I first interpreted the keys and tiara as a skull and crossbones.

I almost second guessed myself because the Saxony coat of arms also has bars with an angled band across.

But I've got a couple of larger Papal States 18th c. coppers that helped keep me headed in the right direction.

Apparently this is not a common type ... couldnt find it either in Numista or NGC.
