I tried to get a snack from the vending machine today and it kept spitting out my quarter. After settling for some snackwells cookies instead of cheezits I went back to my desk and took a closer look at it
I cant tell if it was stamped wrong at the mint or another quarter was somehow fused to it? Any ideas? Here are a few pics
The Washington head is raised up from the rest of the quarter and is smooth to the touch, it does not have any grooves like a normal one

Yes, if those lines on the obverse are raised, you might really have something! Better pictures would help a lot. Looks like a possible struck through error, but too hard to tell at this point. Good luck!
Looks like Glue residue to me like it was glued face down to show the Virginia Quarter Reverse design. The description sounds like it was glued to something as well with it being smooth but I will hold off saying anything concrete until better pictures are up
Bryan1315: You have a fair point, there. I hadn't thought of it. Treatment with organic solvents, depending on the glue type, could answer this question without injury to the coin. Good old acetone is the usual obvious first choice.
