It appears to be a mark on the coin, some Foriegn Substance. Considered Dirt.

Is it just a mark, or is there a change in elevation (raised or incuse)?

i have verified its not any mark by a pen or other writing implement and I am sure it is not PMD because I get it from a roll of UNC 2012-D pennies last october and all in the roll were UNC as well as this one. of course its been exposed to oxygen since then but I have never left it outside or marked it with a pen. it had this mark even when I got it.

I highly doubt this is an error.
I don't see what could cause this in the minting process.

Could be a stain from something that came in contact with the coin. If it is a chemical stain, you won't be able to clean it off. It can happen, even with uncirculated coins.
